It's easy to crash a ship in a narrow channel, there's meant to be lots of mechanisms (both technological and managerial) to prevent this, but they can happen without any grand conspiracy. A crew of 22 semi-competent indians doing a job that requires a White man's competence will get you this kind of disaster, the only weird thing is why it hasn't happened more often.
