I think tongkat ali works by lowering shbg, which could theoretically make testosterone more locally bioavailable. I say locally because shbg bound test allows it to be more easily transported in the blood. So it won't change total test, but it could have some mild anabolic effects depending on your baseline shbg levels.
I naturally have very low shbg levels, so it doesn't do much for me. My total test is generally average, but my free is near double the standard range.
