It happens from visceral fat deposits (fat deposits in the liver known as nafld [non alcoholic fatty liver disease]) that fills up the liver and when the livers full, it begins depositing it in otger organs. Health(ier¡) fat known as subcutanious fat is the one most people have and is stored directly under the skin tho most of those people also sufferfrom nafld justnot the same amount of visceral fat. Most body builders have A1C readings that classify them as diabetic amd thet die of the same causes as obese people, the average life span being 55 years last i heard. Its super common with people usong insulin shots to cram more carbin into thier muscles and using it as an anabolic steroid to keep them from going into catbolism. Get sufficient choline (can reverse afld), don't use C17s or orals (dbol), avoid tren (tren will facilitate balding depending on the stack). Test Erhanate and Cypionate have fairly safe profiles, as does Deca.

Also, test doesn't cause prostate cancer. It will make it worse if you happen to get it though. So there is a "cancer" risk.

Also, steroids are illegal. Don't do them. But be safe if you do.
